Cameron Macchia - APRN
Adult-Gerontology Primary Care Nurse Practitioner
Cameron Macchia is a board-certified Adult-Gerontology Primary Care Nurse Practitioner, holding both a BSN from Chamberlain University and an MSN from the same institution. His early career began as a PCU-Cardiac Nurse at Baptist Downtown’s Heart Hospital, followed by critical care at Baptist South.
Guided by a natural passion for helping others, Cameron is dedicated to delivering top-level care tailored to each patient’s unique needs. He values Jax Housecalls’ mission of meeting patients where they are and believes strongly in the power of compassionate, personalized medicine.
Beyond his professional life, Cameron is a proud father to one daughter. He enjoys traveling, especially cruises, along with golfing and boating.
Christy Carpenter
Operations Director
With over 35 years of healthcare experience, Christy Carpenter brings deep operational expertise and a passion for compassionate care to her role as Operations Director. Before joining Jax Housecalls, Christy led operations at My Choice Housecalls, growing the practice to serve over 3,000 patients across Jacksonville and neighboring counties. Her leadership experience includes hiring, managing, and streamlining systems to improve care delivery.
Christy is a graduate of Jacksonville State University in Alabama and is married to a retired JFRD firefighter. A proud mom of two boys, she enjoys Alabama football, boating, and baseball in her downtime.
What drives Christy every day is the chance to make a real difference in people’s lives. “I go home and I know I made a real difference.” At Jax Housecalls, she finds meaning in supporting a team that genuinely cares—for both patients and one another.
Rachael Shad, APRN, FNP-BC
Family Nurse Practitioner
Rachael Shad is a board-certified Family Nurse Practitioner with a strong clinical background and a heart for home-based care. She began her nursing career at the Mayo Clinic, specializing in neurosciences and medical-surgical units. Over the past six years, she has focused on in-home infusions, providing safe, high-quality care to patients in the comfort of their homes.
Rachael earned her MSN-FNP from Jacksonville University, holds a BSN from the University of Central Florida, and studied Communication at Florida State University.
At Jax Housecalls, Rachael is passionate about delivering care that truly meets patients where they are. “When people are dealing with health issues that make it hard to get out of the house, it’s such a relief for them to still get good care without the stress of traveling.”
Outside of work, Rachael enjoys time with her husband and two daughters, playing tennis, joining her book club, or catching a good podcast.
Bianka Irizarry, FNP-C
Family Nurse Practitioner
Bianka Irizarry is a board-certified Family Nurse Practitioner with a passion for providing compassionate, personalized care. She earned her Master of Science in Nursing from United States University and her Bachelor of Science in Nursing from the University of West Florida. Her diverse background spans inpatient and outpatient care, including primary care, geriatrics, maternity, telehealth, and wellness-focused practices.
At Jax Housecalls, Bianka loves meeting patients where they are—literally and figuratively. She finds fulfillment in supporting healthier, more independent lives through trust-based, home-centered care.
Outside of work, Bianka enjoys traveling the world, spending time at the beach with family and friends, reading, trying new recipes, and, of course, shopping. She is also a proud parent of three adult children, who keep her grounded and inspired.
Djoolcy Dantes, APRN, FNP-BC
Family Nurse Practitioner
Djoolcy Dantes is a board-certified Family Nurse Practitioner who earned both her Bachelor of Science in Nursing and Master of Science in Nursing degrees from Florida Atlantic University. She brings several years of experience across a variety of clinical settings, caring for patients across the lifespan, which has shaped her well-rounded and compassionate approach to patient care.
Djoolcy’s care philosophy centers on treating the whole person, prioritizing open communication, patient education, and individualized care plans. At Jax Housecalls, she enjoys delivering high-quality, personalized care while building meaningful relationships with patients in the comfort of their own homes.
Outside of work, Djoolcy enjoys staying active, focusing on health and wellness, and spending quality time with family and loved ones.
Estera Vatamaniuc, FNP-BC
Family Nurse Practitioner
Estera Vatamaniuc is a board-certified Family Nurse Practitioner committed to delivering comprehensive, patient-centered care across all ages. She specializes in family practice and is experienced in diagnosing, prescribing, and managing treatment plans for a wide range of conditions. Estera is particularly passionate about preventive care and health education, helping patients take proactive steps toward long-term wellness.
Before entering family practice, Estera built a strong foundation in critical care while working in the Intensive Care Unit. She holds a Master of Science in Nursing from Chamberlain University and a Bachelor of Science in Nursing from Loyola University Chicago.

Levan Carter, ARNP, PMHNP-BC
Levan is a board-certified Psychiatric-Mental Health Nurse Practitioner dedicated to providing compassionate and patient-centered mental health care. She earned her Bachelor of Science in Nursing from Grand Canyon University and her Master of Science in Nursing from Spring Arbor University. She is currently enrolled in the Doctor of Nursing Practice program at Spring Arbor University, where she is expected to complete her degree in September 2026.
Levan’s nursing background includes serving as a charge nurse in correctional health and providing psychiatric care at New Horizons of the Treasure Coast. She has cared for diverse populations, including geriatrics, adolescents, and children, and has completed over 590 clinical hours in psychiatric and mental health settings.
Her professional interests include medication management, anxiety and mood disorders, and holistic approaches that integrate mind, body, and spirit. Levan strongly believes in being a voice for her patients and will advocate for them to the fullest, ensuring that each individual receives the highest quality of care and support on their journey to wellness.
